Your Physical Therapy Recovery Journey

Discover the path to lasting recovery with our personalized, multidisciplinary care approach.

  1. Comprehensive Assessment

We evaluate your condition, review your medical history, and run the necessary movement and neurological exams to understand exactly what's driving your pain.

  2. Personalized Treatment

You begin a customized care plan — chiropractic adjustments, hands-on physical therapy, and acupuncture as needed — designed for your case and your goals.

  3. Progress Monitoring

We re-assess your response to treatment every two to three weeks and adjust the plan to keep your recovery on the fastest, safest path.

  4. Long-Term Wellness

As you progress, we shift focus to maintenance — home exercises, posture, and lifestyle strategies that keep the gains and prevent flare-ups.

Physical therapy is what happens between the injury and the life you want to get back to. Whether you tore something on the soccer field, woke up with a stiff back that never quite loosened up, or came home from surgery with a long list of exercises and no idea how to do them safely — our team in Morganville builds a plan that meets you where you are and moves you forward, one visit at a time.

Led by Dr. Robert Perniola, DPT — a New Jersey board-certified physical therapist with training in McKenzie Method, orthopedic rehabilitation, and sports medicine — our PT program is built around hands-on care and progressive, measurable exercise. We don’t hand you a sheet of exercises and send you to a corner. You work alongside your therapist on every visit, and the plan evolves as your body does.

Our approach to physical therapy

Good physical therapy starts with a thorough evaluation and ends with you not needing us anymore. In between, it should be specific, hands-on, and honest about progress.

We combine four elements on nearly every case:

  • Manual therapy. Joint mobilization, soft-tissue work, myofascial release, and targeted stretching to restore motion and quiet irritated tissue.
  • Corrective and therapeutic exercise. Progressive loading that rebuilds the strength, stability, and endurance you need to return to your actual life — not just pass a range-of-motion test.
  • Movement retraining. Teaching your body to move better under the load that caused the problem in the first place, whether that’s a deadlift, a stair, or picking up your grandchild.
  • Education. You should leave every visit knowing what’s happening, what’s improving, and what you can do at home. A patient who understands their own case gets better faster and stays better longer.

We also work shoulder-to-shoulder with our chiropractic and acupuncture teams under the same roof. If your back pain would respond faster with a chiropractic adjustment or acupuncture between PT visits, that conversation happens at the front desk, not across town.

Conditions we treat with physical therapy

Physical therapy is one of the most versatile tools in musculoskeletal medicine. In our Morganville clinic, we regularly treat:

  • Low back and neck pain — acute strains, disc-related pain, and chronic postural pain
  • Sciatica and radiating nerve pain
  • Shoulder problems — rotator cuff tears and tendinopathy, impingement, frozen shoulder, post-surgical rehab
  • Knee pain — meniscus injuries, patellofemoral pain, ACL rehab, arthritis, post-surgical recovery
  • Hip, foot, and ankle pain — including plantar fasciitis and recurrent ankle sprains
  • Elbow, wrist, and hand conditions — tennis and golfer’s elbow, carpal tunnel, post-fracture stiffness
  • Post-operative rehabilitation — rotator cuff repair, joint replacement, spinal surgery, ACL reconstruction, and more
  • Sports injuries at every level of competition
  • Neuropathy and balance dysfunction
  • Auto injury and workers’ comp cases, with full administrative handling on our side

If you’re not sure whether PT is the right starting point, call us at (732) 972-6010. Our team will help you figure out the best entry point — which may be physical therapy, may be chiropractic, or may involve both.

What to expect on your first visit

Your first visit with Dr. Perniola is a true evaluation, scheduled for enough time that we can actually answer your questions and build a real plan.

  1. History and goals. We ask about onset, aggravating and relieving activities, medical history, prior imaging, and — most importantly — what you want to get back to doing.
  2. Objective testing. Posture, range of motion, strength, neurological screening, balance, and movement-quality tests that relate to your specific problem. When it’s relevant, we use our computerized range-of-motion system to capture precise baseline numbers.
  3. Diagnosis and explanation. You learn what’s happening in plain English, and we review the findings with you on a whiteboard or model. No mystery, no upsell.
  4. Initial treatment. Most first visits include hands-on manual therapy and an exercise or two you can start right away.
  5. Plan. You leave with an expected visit count, a home program calibrated to your schedule and equipment, and clear progress markers so you’ll know whether the plan is working.

Why integrated care matters

A lot of back and joint pain has more than one driver. A stiff hip joint can keep strength work from taking hold. A compressed nerve can keep manual therapy from sticking. Chronic muscle guarding can make every adjustment feel like it “only lasts a few days.”

That’s why our chiropractic and PT teams function as a single care team. Dr. Perniola and Dr. Rick Caban read each other’s notes, adjust each other’s plans, and will often hand you off mid-visit when something benefits from a different set of hands. Our licensed acupuncturist complements both services for patients whose pain flares with stress, who have stubborn muscle tension, or who want a drug-free adjunct to their rehab.

You get more progress per visit, fewer appointments on your calendar, and a team that actually talks to each other about your case.

Home program and long-term maintenance

We are not trying to keep you on a visit schedule forever. Every patient leaves with a home program — usually 10 to 20 minutes a day — that’s short enough to actually do and specific enough to matter. We progress it together, and we take things off the list once you’ve earned it.

When you graduate from active care, we’re still here. A single tune-up visit every few months is enough for many patients to keep small flare-ups small, and our front desk will always find you a spot when something new crops up.

Physical Therapy — Frequently Asked Questions

Do I need a doctor's referral to start physical therapy in New Jersey?

In most cases, no. New Jersey is a direct-access state, which means you can begin a physical therapy evaluation with us without a referral. Some insurance plans still require a prescription for coverage, so when you call (732) 972-6010 we'll verify your specific plan and let you know before your first visit.

How is physical therapy different from chiropractic care?

Chiropractic focuses on joint motion and nervous-system function, often through manual adjustments. Physical therapy focuses on rebuilding strength, mobility, and movement patterns through hands-on techniques and progressive exercise. They solve different pieces of the same puzzle, which is why so many of our patients see both providers in a single visit.

How long will I be in physical therapy?

Most orthopedic cases resolve in 6 to 12 weeks of twice-weekly visits, though acute injuries often need less and post-surgical cases often need more. At your evaluation, Dr. Perniola will give you an honest estimate based on the specifics of your case rather than a generic protocol.

Will physical therapy hurt?

You should expect some hard work, not suffering. We scale every exercise and manual technique to your current tolerance and progress it only as your body is ready. Mild soreness for a day or two after a session is normal; sharp or lasting pain is not, and we adjust the plan if it shows up.

Can you help after orthopedic surgery?

Yes. Post-surgical rehab is one of our core specialties. We work from your surgeon's protocol and customize it to how your body is actually recovering — rotator cuff repairs, ACL reconstructions, total joint replacements, spinal surgeries, and more.

Do you treat athletes and active patients?

Regularly. Dr. Perniola has a strong sports-medicine and running-mechanics background, and we see everyone from middle-school athletes to weekend runners to older adults who want to keep their hiking, tennis, and pickleball habits going strong.

Do you accept my insurance?

We accept most major plans including Horizon BCBSNJ, Aetna, Cigna, and UnitedHealthcare. We also handle auto injury (PIP) and workers' compensation cases. Call (732) 972-6010 and we'll verify your benefits before you come in.

Ready to start feeling like yourself again?

Same-day appointments are usually available. Most insurance accepted. Call or schedule online today.
